Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to structural collapse of the workover rig: a) On or about August 27, 2008 and at times prior Purgatoire Valley Well Service did not ensure that employees were protected from crushing hazards due to structural collapse. Proper inspections and preventative maintenance were not being performed on the workover rig (#10). Among other methods one feasible and accepable abatement method to correct this hazardous condition would be to develop and implement procedures for inspection, maintenance, and repair of rig structures that conforms to American Petroleum Institute Recommended Practices 4G, Third Edition, April 2004. Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").-Certification

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to burns in the event of a rig fire while working from an elevated work platform without an emergency escape line installed: a) On or about August 27, 2008 and at times prior Purgatoire Valley Well Service did not ensure that employees were protected from fire/explosion hazards. No auxiliary means of excape was installed for personel working in the derrick. Among other methods, a feasible and acceptable method to correct this hazard would be to provide a means of escape (such as a Geronimo system) as specified by American Petroleum Institute Recommended Practice 54 Section 6.10.1 of 1999. Abatement Note: Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").-Certification

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards from improper application, care, and use of wire rope: a) On or about August 27, 2008 and at times prior, Purgatoire Valley Well Service did not ensure that employees were protected from wire rope hazards. 1. Hoisting and standing lines had broken wires, bird caging, kinking, and crushing. 2. Clamps were not installed correctly in that they were not properly sized, spaced, and torqued. Among other methods, a feasible and acceptable method to correct this hazard is to follow the recommendations of American Petroleum Institute Recommended Practices 54 and 9B Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").
