Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment or a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries: a)On or about September 10, 2008, and at times prior, employees in the plasma shop were exposed to crushing injuries when operating the Nissan forktruck which did not have a seat belt. Among other methods, a feasible and acceptable method of correcting this hazard would be to install seat belts on all forktrucks. Abatement Note: Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment or a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards: a)On or about September 10, 2008, employees in the yard were exposed to crushing injuries where the John Deere 410B backhoe was operated without a backup alarm. Among other methods, a feasible and acceptable method of correcting the hazard would be to either repair or replace the existing backup alarm if it is not operational, or to install a new backup up alarm. Abatement Note: Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").
