a. On or about January 6, 2009, and at times prior, employees were exposed
to
overhead/crushing hazards from an open/accessible space beneath the
freight elevator. The
freight elevator controls are on the upper level and do not allow the
operator to see beneath
the elevator while it is being lowered.
Abatement Note:  Abatement certification and documentation is required for
this item
(see enclosed "Sample Abatement-Certification Letter").
Among other methods, one feasible and acceptable abatement method to
correct this
hazardous condition would be to have a interlocked gate, which restricts
access to the area
beneath the freight elevator when it is in the raised position and/or
being
lowered.
Additionally, the interlock would shut off power to the elevator when it
is opened.

storage cellar door
was operational.
Among other methods, one feasible and acceptable abatement method to
correct this
hazardous condition would be to repair the handle to ensure employees
could exit the
refrigerated beer cellar if the door were to close behind them.
