Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish to each of his employees employment or a place of employment which
were free
from recognized hazards that were causing or likely to cause death or
serious physical harm
to employees in that employees were exposed to potential structural
collapse of the rig:
a)On or about January 14, 2009, and at times prior, employees were exposed
to
crushing injuries where the wire rope clamps used to anchor the rig on the
southwest
corner were not properly attached.  The clamps were attached with the dead
end of
the rope resting in the saddle;
b)On or about January 14, 2009, and at times prior, employees were exposed
to
crushing injuries where on the rig anchor positioned at the southwest
corner of the
rig, the wire rope clamps were not attached as close to the shiv/loop as
possible.
Among other methods, one feasible and acceptable method to correct this
hazardous
condition would be attach the clamps in accordance with manufacturer's
recommendations as
well as with Figure 6 of Sections 3.7.4 and 3.7.4.2 of the American
Petroleum Institute
Recommended Practices on Application, Care, and Use of Wire Rope for Oil
Field Service,
API RP 9B.

a)On or about January 14, 2009, and at times prior, employees on the drill
floor were
exposed to being struck by drill pipe and/or other hoisted material where
to wire rope
in use for the sand winch was damaged and in need of replacement.
Among other methods, one feasible and acceptable method to correct this
hazardous
condition would be to replace the wire rope in accordance with the
American Petroleum
Institute Recommended Practices 9B.
