Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm to employees in that, employees were exposed to struck-by hazards by the mast. a) On or about February 11, 2009, and times prior, the employer did not ensure that employees were protected from a falling mast where the wire rope clips on the guy wires of Rig #34 and the auxilary hoist were of the wrong size. Size 7/16 inch clamps were used on 1/2 inch cable. One feasible and acceptable abatement method among others is to use the correct size of wire rope clips for the size of the wire cable according to the manufacturer's specifications and according to American Petroleum Institute Recommented Practices (API-RP 9B-3.3.8). Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm to employees in that, employees were exposed to struck-by hazards by the tongs. a) On or about February 11, 2009, and times prior, the employer did not ensure that employees were protected from falling tongs where the cotter pin which holds the tong in place was damaged and halfway out. One feasible and acceptable abatement method among others is to develop and implement procedures for inspections, maintenance, and repair of work over rig structures that conforms to American Petroleum Institute Recommented Practices (API-RP 4G) Third Edition, April 2004. Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").
