Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm from being struck by in that, the clamps on the guy lines were not spaced properly: a) On or about March 19, 2009 and at times prior, employees were exposed to injury while operating Rig #5 in that, the clamps on the guy lines wire rope were spaced 9-21 inches apart. Among other methods, one feasible and acceptable method to correct this hazard condition would be to install the clamps in accordance with Table 2 of Section 3.7.4 of the American Petroleum Institute Recommended Practice on Application, Care and Use of Wire Rope for Oil Field Service, API RP 9B. Abatement Note: Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").
