a) On or about March 23, 2009 and times prior, employees were exposed to
serious injuries
from being crushed by the backhoe bucket.  During the operation of the JD
200 CL backhoe,
hand signals or other forms of communication were not used for direction
of work.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to provide radios to be used when employees are out of the line of sight
of the operator.

required for this
