Section 5(a)(1) of the Occupational Safety and Health Act of 1970; The
employer did not
furnish employment and a place of employment which was free from
recognized hazards
which were likely to cause death or serious physical harm to employees in
that, employees
were exposed to electrical shock hazards:
a) On or about May 11, 2009 and at times prior, employees were exposed to
electrical
hazards while working with the following pieces of equipment;
1) The Piler, serial #1466, was missing a cover and dead front on a
breaker box
located on the side of the main control panel
2) The Scoopie, serial #245;
i) The box for the directional belt switch was damaged and being held
together
with a hose clamp.
ii) A junction box was not installed below the direction belt switch for
the
wires that had been spliced.
3) The Scoopie, serial #400;
i) A 220 volt cable was not provided with a bushing at the point it
entered a
metal box.
ii) The flexible conduit was damaged in two places exposing the inter-
conductors to damage.
Among other methods, a feasible and acceptable method to correct these
hazards would be to
develop and implement an inspection and maintenance program for all
equipment to help
ensure the identification and repair of all defects prior to use of
equipment.
