Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushed-by hazards: On or about 04/22/08, at 17250 SW 192nd Street, employees that were assisting in testing an Altec aerial lift were exposed to the hazard of being crushed by the equipment. Among other methods of abatement, one feasible and acceptable method to correct this hazard is to ensure that employees follow safe work practices as recommended in the Altec Industries Operator's Manual and operate the functions from the lower controls and have no one in the platform. PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
