On or about 04/22/08, at 17250 SW 192nd Street, employees that were
assisting in testing an
Altec aerial lift were exposed to the hazard of being crushed by the
equipment.
Among other methods of abatement, one feasible and acceptable method to
correct this hazard
is to ensure that employees follow safe work practices as recommended in
the Altec Industries
Operator's Manual and operate the functions from the lower controls and
have no one in the
platform.
PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS
VIOLATION.
