General-duty citation text
Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees riding an Ottawa tractor during transfer movements were exposed to crushing hazards. On or about 05/24/2008, at 7000 NW 60th Street, in Miami, Florida, an Ottawa tractor trainer was standing on an Ottawa tractor platform and connected pneumatic lines onto a trailer as a trainee reversed the Ottawa tractor onto the trailer. Among other methods of abatement, one feasible and acceptable method to correct this hazard is to comply with the manufacturer's warning of never allowing anyone to ride outside of the cab. PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
