A.  On September 16, 2008, employer knew or should have known that four
employees were
exposed to the hazards of being backed over by trash truck, Tag Number
G39602, when it
was backing up without their knowledge of its reverse movement.  A
feasible and useful
means of abatement, among others, is to install an operating back-up alarm
on the Ford trash
truck, Tag Number G39602, in the yard at the maintenance shop for roads
and bridges.
