A.  Employer knew or should have known that an employee operating a

Model 6042, with inoperable reverse signal alarm constituted a hazard of
employees being
struck by forklift when it is being operated in reverse gear at the
Brownstones Townhouse
job site.  A feasible and useful method, among others, to correct this
hazard, is to ensure the
reverse signal alarm is properly working.
