recognized hazards
which may cause death or serious physical harm to his employees and comply

regulation and other occupational safety and health rules and regulations
promulgated under

up alarm and no
observer used constituted a hazard of employees being backed over and
crushed.  A feasible
and useful method, among others, to correct this hazard is to ensure
forklift has an operable
back up alarm when in reverse at the SCANA gas training facility job site.
