Daewoo,
Model #GCZ5E-3, Serial Number JZ-00317, forklift, without wearing a seat
belt was
exposed to the hazard of being thrown from the driver's seat.  A feasible
and useful means
of abatement, among others, is for employee to use the seat belt.
