A.  Employer knew or should have known that no seat belt provided for a
Lull 644TT-34
forklift, Serial Number JD2644W, used on job site constituted a hazard of
operator being
thrown from the forklift.  A feasible method, among others, to correct
this hazard is to
provide forklift with seat belt and to wear seat belt when operating
forklift.
B.  Employer knew or should have known that no back-up alarm for Lull
644TT-34 forklift,
Serial Number JD2614W, used on job site constituted a hazard of employees
being backed
over.  A feasible method, among others, to correct this hazard is to equip
forklift with an
operable back-up alarm.
