Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire hazards: (a) Location - Elevator shaft by stair C: On or about 6/25/08, the employer failed to ensure that a 6-story elevator shaft had fire- rated door assemblies at all floor-level access points. Among other methods, one feasible and acceptable method to correct this hazard is to install fire doors at all access points to the elevator/shaft in accordance with NFPA 101 Life Safety Code, 2006, table 8.3.4.2.
