crushed by a powered industrial truck in the event of an upset:
a)establishment - Toyota forklift was not equipped with a seatbelt.  An
employee
was observed operating the forklift without a seatbelt which could result
in the
employee being thrown from the lift truck in the event of a vehicular
accident.
ABATEMENT NOTE:  One feasible and acceptable method to correct this hazard
is to
retrofit the forklift with a seatbelt.
