- Manufacturers Operation & Maintenance Manuals for SP84 lifts (Bay#1,
Bay#5) and
Weaver lift, model AFH-70-B 5AP-1007 (Bay#3) were not readily available
for review
by employees, to be used for training purposes, and for guidelines to
perform periodic
inspection/maintenance procedures.
- There was evidence of lack of maintenance on lifts such as, but not
limited to:
a) On or about 05/13/08, in the service area, Bays #4 and #7, a latch
release air button
(lift model SM122-100) was damaged and a rolling jack switch was not
functioning.
b) On or about 05/13/08, in the service area, Bay # 6, a lift's hydraulic
line was
leaking, and a cloth rag was used to control the leak.
c) On or about 05/13/08, employer did not provide formal training to
technicians/
mechanics on the safe operation of automotive lifts as required by
ANSI/ALI
ALOIM-
2000 and manufacturers automotive lifts manuals.
Among other methods, feasible and acceptable abatement methods to correct
these
hazards include, but are not limited to the following:
1.Conform to the American National Standards Institute/Automotive Lift
Institute
(ALI) safety requirements for operation, inspection and maintenance,
ANSI/ALI ALOM-
2000, Sections 4, 5, and 6.
2.Develop and implement standard operating procedures for hydraulic lifts
which
include daily, periodic and annual inspections and a preventative
maintenance program
by qualified personnel in accordance with manufacturers specifications as
outlined in the
ANSI/ALI ALOIM 2000.
3.Provide and document training to all employees on the safe operating
procedures
of all hydraulic lifts, including but not limited to the operation,
maintenance, daily and
periodic inspections.4.Ensure all lifts are rated and marked with their
load capacity.
5.The employer shall display in a visible location of the lift area
general safety
information, safety tips and warning labels with each lift manufactured in
accordance
with ANSI/ALI ALOIM-2000.
6.Remove defective lifts from service until repaired and in safe working
condition.
