were causing or likely to cause death or serious physical harm to

were exposed to the hazard of falling to the ground while on a wood pallet
which was being
raised in the air by the forks of a powered industrial truck:
a) On or about 6/13/08, in the warehouse area between shelving: An
employee was
kneeling on a wood pallet which was being raised in the air by the forks
of a Toyota
propane powered industrial truck. The employee was preparing to put some
items on the
3rd level of industrial shelving when she fell off the pallet. The wood
pallet was not
secured to the truck, lacked guardrails, and was not an approved work
platform.
ABATEMENT CERTIFICATION REQUIRED
Among others, a feasible method to eliminate or materially reduce this
hazard is to:
1. Provide a properly engineered "cage" type work platform with guardrails
installed,
and securely fasten it to the carriage of the forklift truck.
