Employer failed to implement a procedure to ensure that the swing radius
and the rear path of
travel of the Hyundai tracked excavator was protected by a back up guide
or cordoning off the
path of travel because of the obstructed vision of the operator. Violation
occurred on or about
6/27/08.
Abatement notes: Among other methods, feasible and acceptable methods to
reduce this hazard
are to;
a) Follow the procedures in the operator's manual for the Hyundai 160LC-7
i.e. Operate
carefully to make sure all personnel are clear within the working range of
the machine.
b) Place barrier guards if necessary.
c) Assign and use a ground guide.
Violation occurred on or about 6/27/2008.
