Nevada Revised Statute 618.375(1): Duties of employers. The employer failed to f urnish employment and a place of employment which was free from recognized hazar ds that were causing or likely to cause death or serious physical harm to his em ployees: The employer did not ensure that safe practices and operations of a sel f-propelled elevating work platform were followed, as follows: a.At Mike's Food Mart, 1196 East Tropicana Avenue,Las Vegas, NV 89119, south side of canopy over gas pumps, three employees were exposed to a fall of approximately 20 feet while vacating and entering a scissor lift (make: Skyjack, model no: SJIII 3219, equi pment ID no: 59353) to gain access to and egress from a 20 foot tall canopy to p erform electrical wiring of LED lights on the east and west ballasts. Reference: ANSI/SIA A92.6-1990 Self-Propelled Elevating Work Platforms. Section 8 Responsibilities Of Users, Paragraph 8.10.1, Personnel Footing: Person nel shall maintain a firm footing on the platform floor while working thereon. T he use of planks, ladders, or any other devices on the platform for achieving ad ditional height or reach is prohibited. One feasible means of abatement is to re train operators of scissor lifts in proper safety procedures recommended by the self-propelled elevating work platform manufacturer.d
