Nevada Revised Statute 618.375(1):  Duties of employers.  The employer
failed to furnish
employment and a place of employment which was free from recognized
hazards that were
causing or likely to cause death or serious physical harm to his employees:
The employer failed to ensure that exposed employees adhered to the safe
practices and
operations of a self-propelled elevating work platform, or scissor lift,
as follows:
a.At Arroyo Marketplace Major A, 7280 Arroyo Crossing Parkway, Las Vegas,
NV 89101, an employee was exposed to a fall of approximately 18 to 20 feet
while standing on toprails and planks that had been placed through the
midrails
of a self-propelled elevating work platform, or scissor lift for achieving
additional
height or reach.

Section 8 Responsibilities Of Users, Paragraph 8.10.1, Personnel Footing:
Personnel shall maintain a firm footing on the platform floor while working
thereon. The use of planks, ladders, or any other devices on the platform
for
achieving additional height or reach is prohibited.
One feasible means of abatement is to retrain operators of scissor lifts
in proper
safety procedures recommended by the self-propelled elevating work platform
manufacturer.d
