Nevada Revised Statute 618.375(1):  Duties of employers.  The employer
failed to furnish
employment and a place of employment which was free from recognized
hazards that were
causing or likely to cause death or serious physical harm to his employees:
The employer did not ensure that the safety precautions of the
manufacturer's self-propelled
elevating work platform, or scissor lift were followed, as follows:
a.At the RTC complex construction site located at 5165 West Sunset Road,
Las
Vegas, NV 89118, paint booth installation activities, two employees were
exposed
to fall hazards of 22 feet while vacating and entering scissor lift #2
(make:
Skyjack, SJII series, model no: 6832, serial no: 80434). The employees were
using the lift to access and egress the roof of a newly constructed paint
booth to
tighten nuts onto bolts that were fed through the ceiling.

elevating aerial platform is in the fully retracted position".
Left column, first bullet below the warning box: "Do Use Three Point
Contact
To Enter And Exit The Platform. Enter and exit the platform from the ground
only. Face the machine when entering or exiting the platform.
One feasible means of abatement would be to train scissor lift
operators/workers
in the proper safety procedures as required by the manufacturer of the
self-
propelled elevating work platform and ANSI/SIA 92.6-1990.
