ximately 20 feet while working from a personnel platform, which was attached to
a rough terrain forklift that did not have an operator in the operator's positio
n. The personnel platform was not equipped with controls to operate the platform
. The forklift wheels were not blocked in the absence of the operator while the
two
workers were elevated. Reference: ANSI/ITSDF B56.6-2005 Safety Standard for Roug
h Terrain Forklift Trucks Section 5.15 Elevating Personnel, Paragraph 5.15.1(s)
a trained operator shall be in position to control the rough terrain forklift tr
uck, or available to operate controls if the platform is not equipped with contr
ols. When the operator is not in the operating position, block the truck wheels
and apply the parking brake with all travel controls in neutral. One feasible me
ans of abatement would be to maintain the presence of a trained operator when us
ing a personnel platform as an attachment to a rough terrain forklift as require
d by the above standard. The employer failed to ensure that employees maintained
firm footing on the floor of a personnel
platform, as follows: b.At 3445 Pama Lane, Las Vegas, NV 89120, roofing activiti
es, Employee #1 was exposed to a fall hazard while working at an elevation of ap
proximately 20 feet. The employee climbed onto the roof from a personnel platfor
m, which was attached to a rough terrain forklift, so he could climb down from t
he roof via extension ladders for purposes of transporting roofing material to t
he ground and/or moving the forklift to a different location. Reference: ANSI/IT
SDF B56.6-2005 Safety Standard for Rough Terrain Forklift Trucks Section 5.15 El
evating Personnel, Paragraph 5.15.1(w) platform shall be lowered to floor level
for personnel to enter and exit. Personnel shall not climb on any part of the ro
ugh terrain forklift truck in attempting to enter and exit. One feasible means o
f abatement would be to train employees in the proper use
of personnel platforms as required by the standard above.

d not been inspected prior to use. The SS identified the personnel platform to b
e missing a bolt and nut at one corner where the railing members were being held
together by wire, four of five bolts were each missing a nut to keep the railin
g members from coming apart, and one bolt was observed to pass through the holes
of the railing members only partially. Employee #1 climbed onto the roof from
the personnel platform to make his way to the ground via extension ladders. Refe
rence: Star Industries Operators Manual and Safety Instructions for Star Safety
Work Platforms, Safety Recommendations 4) Inspect complete platform and guardrai
l assembly for rust, damage, broken welds, or missing parts. Check that safety d
ecals are installed and legible. If any abnormality is found, discontinue use an
d contact a Star dealer for a safety inspection. Do not make any modifications t
o the platform without manufacturer's written approval. One feasible means of ab
atement would be to require employees to properly inspect personnel platforms pr
ior to use as required by the standard above.
