Section 6(a) of the Puerto Rico Occupational Safety and Health Act: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or physical harm to employees in that employees were exposed to: Seccion 6(a) de la Ley de Seguridad y Salud en el Trabajo de Puerto Rico: El patrono no proveyo empleo y un sitio de empleo libre de riesgos reconocidos que esten causando o que puedan causar muerte o dano fisico a los empleados, por cuanto empleados estaban expuestos a: At construction site, Mc Donalds Project - elevated work platform mounted on rough terrain power industrial truck did not comply with the following ANSI B56.1-2000 requirements: a. The platform did not have capacity ratings. b. The platform was not properly secured to the forks. c. The platform sides did not have an intermediate guardrail. An elevated work platform with dimensions 51" wide, 96" long, and 40" high was installed in the forks of a Rough Terrain Forklift Genie GTH-644. Employees stand on platform at aproximately 15 feet high to install and remove molds, among other tasks. Among other methods, one feasible and acceptable abatement method to correct this hazard is: a. The work platforms shall have permanent, legible identification providing information of platform weight, platform rated load, name and address of the manufactuer or name of engineer with engineering certification. b. The work platform shall be secured to the fork carriage to prevent forward, lateral or rotational movement. c. The platform shall have an intermediate rail and a 4 inch high toe board on all sides. Hazard: Falls. En el sitio de la construccion - plataforma elevada para trabajo montada en montacarga tipo todo terreno no cumplia con los siguientes requerimientos del ANSI B56.1-2000: a. La plataforma no tenia la clasificacion de capacidad. b. La plataforma no estaba asegurada a los tenedores en forma apropiada. c. Los lados de la plataforma no tenian una baranda intermedia. Una plataforma de trabajo elevada con dimensiones de 51" de ancho, 96" de largo y 40" de alto estaba instalada en los tenedores del Montacarga de Todo Terreno Genie GTH-644. Los empleados se paran en la plataforma a alturas de aproximadamente 15 pies para instalar y remover moldes, entre otras tareas. Entre otros metodos, un metodo factible y aceptable de corregir este riesgo es: a. La plataforma de trabajo debe tener identificacion permanente y legible, proveyendo informacion del peso de la plataforma, clasificacion de carga de la plataforma, n ombre y direccion del manufacturero o el nombre del ingeniero con la certificacion de ingenieria. b. La plataforma de trabajo debe asegurarse a los tenedores para evitar movimientos frontal, lateral, o en rotacion. c. La plataforma debe tener baranda intermedia y 4 pulgadas de alto de tabla de capellada en todos los lados. Riesgo: Caidas.
