hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to:
On or about October 1, 2008, at the job site located at 492 Highway 25,
North, Guy,
Arkansas, an employee installing bar joists from the platform of a scissor
lift while a
Skytrak operator lowered bar joists in place, was exposed to the hazards
of being struck
by the load as it was being lowered above his head.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to have the employee move the scissor lift from beneath the load until it
rested on the supports
before performing welding operations.

employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to:
On or about October 1, 2008, at the job site located at 492 Highway 25,
North, Guy,
Arkansas, the employer did not use an approved lifting attachment when
lifting a free
rigged bar joist via a sling placed over the tines of a Skytrak 8042 for a
below the tines
lift, exposing employees to the hazards of being struck by the load in the
event it slipped
off the forks.
Among other methods, one  feasible and acceptable abatement method to
correct this hazard is
to (1) ensure that an  approved lifting hook or other attachment approved
by the manufacturer
is used when lifting a load and (2) use the pins provided on the
attachment to secure the lifting
hook behind the heel of each fork to protect against the attachment being
disengaged
from the
forks.

platform extended
to approximately 11 feet above the level below, riding in the scissor lift
from one
location to another while the access gate was not closed, exposing the
employee to the
hazards of a fall from elevation.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard is
to ensure the gate is closed when the self-propelled elevating work
platform is in the raised
position as required by ANSI/SIA A92.6-1990, 7.9 During Operations,  item
(3) which states
"The guardrails are installed and access gates or openings are closed per
manufacturer's
instructions."

open sides and ends
of platforms:
