employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to THE HAZARD OF BEING STRUCK-BY falling loads:
a)Chopper Area, on or about 3/26/09:  2-ton Gorbel hoist had splices in the
pendent cable and the pendent controls were not labeled.
b)Gerwin Machine, on or about 3/26/09:  3-ton CM hoist had splices in the
pendent cable and the support structure was not marked with a load capacity
rating.
c)Synco Machine Area, on or about 3/26/09:  2-ton CM hoist had a missing
safety latch on the load hook and the support structure was not marked with
a load capacity rating.
d)Bull Block Area, on or about 3/26/09:  3-ton CM hoist had the control
pendent broken in half with tape holding it together.
Among other methods, one feasible and method of abatement to correct these
hazards is to conduct frequent and periodic inspections of lifting devices
for wear,
deterioration and malfunction, make the necessary repairs or replacements
when
needed, and post load capacity ratings as per ANSI B30.20-1993.
