Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of a fire/ explosion: a)On or about 3/19/09, Crouch 4H Well: Well control was not maintained at all times. There was no Ram style Blow Off Preventor (BOP), just an Annular BOP. Among other methods, one feasible and acceptable abatement method to correct this hazard is to provide Blow Of Prevention that will allow for well control to be maintained at all times as per API RP54, Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations, section 6.1.
