Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck by the load should the overhead
hoist, below-the-
hook lifting devices or bridge crane system fail:
a)The following overhead hoists did not have the rated load markings on the
hoist, the pendant controls were not labeled as to what they controlled and
they were not inspected:
1.Machining Dept., on or about 5/28/09:  A hand chain hoist at the
Mazak AJV-18 vertical milling center had no rated load capacity
markings on it and had not been inspected since March, 2008.
2.Machining Dept., on or about 5/28/09:  The pendant control for the
50 Ton Spacemaster overhead crane at the Mazak V40 machining area
did not have the control buttons labeled as to what they controlled.
Among other methods, feasible and acceptable abatement methods to correct
these
hazards are to:
1.Clearly mark the rated load on the hoist in accordance with ANSI B
30.16-1993, Section 16-1.1.1
2.Inspect the hoists in accordance with ANSI B30.16-1993, Section 16-
2.1
3.Mark the pendant controls as to what they control in accordance with
ANSI B30.16-1993, Section 16-2.3.
b)The following bridge crane structures had no rating on the bridge crane
structure and the supporting structure for the hoist was underrated for
what
the hoist capacity was rated for:
1.Machining Dept., on or about 5/28/09:  A bridge rail with a hand
chain hoist attached at the Mazak AJV-18 vertical milling center had
no rated load capacity markings on it.
2.Linear Motor Cell, north side of oven, on or about 5/28/09:  A one ton
chain hand hoist rated for one ton was mounted to a jib crane that
was rated for 250 lbs.
Among other methods, feasible and acceptable abatement methods to correct
this
hazard is to:
1.Prior to installation and use, ensure the supporting structure is
designed to withstand the loads and forces imposed by the hoist for
the rated load in accordance with ANSI B30.11-1980, Section 11-2.2.2.
2.Mark the rated load on the bridge in accordance with ANSI B30.11-
1980, Section 11-1.1.
c)The following below-the-hook lifting devices were not inspected and/or
had
no rated load capacity markings on them:
1.Linear Motor Cell, on or about 5/28/09:  Two below-the-hook lifting
devices at the entrance and exit ends of the ultra sonic parts cleaner
were not rated and had not been inspected.
2.Linear Motor Cell, on or about 5/28/09:  A below-the-hook lifting
device attached to a one ton chain hand hoist at the north side of the
oven had no rated load markings on it and had not been inspected.
Among other methods, feasible and acceptable abatement methods to correct
these
hazards are to:
1.Inspect the lifting devices in accordance with ASME B30.20-1993,
Section 20-1.3.
2.Mark the rated load capacity on the device along with the
manufacturer's name, serial number and lifter weight, if over 100
lbs., in accordance with ASME B30.20-1993, Section 20-1.2.
Abatement certification must be submitted for these items.
