a)Steel storage area, on or about 07/22/2009, a Coffing 1-ton overhead
electric
hoist had a hook safety latch removed, the lower limit safety switch did
not
function in accordance with the manufacturer's manual and no recorded
annual inspection had been conducted.
b)Steel storage area, on or about 07/22/2009, a Lynx 1-ton overhead
electric
hoist had the hook safety latch removed, the lower limit safety switch did
not
function in accordance with the manufacturer's manual and no recorded
annual inspection had been conducted.
c)Accupress area, on or about 07/22/2009, A Lynx 1-ton overhead electric
hoist
had a lower limit safety switch that did not function in accordance with
the
manufacturer's manual and no recorded annual inspection had been
conducted.
Among other methods, several feasible and acceptable abatement methods to
correct
this hazard are to:
1.Repair or replace the safety latch and ensure the Safety limit switches
are
adjusted to meet the requirement of the manufacturer's operation manual,
Section: Checking Limit switch operation and ANSI B30.16.-1993, Overhead
Hoists, Section 16.2, Adjustments, Repairs, and Replacements.
2.Conduct frequent and periodic inspections of the hoists in accordance
with
ANSI B30.16-1973-1993 Overhead Hoists, Section 16-2.1 Inspection.
d)Steel Storage area, on or about 07/22/2009, The two (2) Overhead hoist
monorails did not have rated load tests conducted and maintained on file
and
no recorded annual inspection had been conducted and maintained.
e)Accupress area, on or about 07/22/2009, One (1) Overhead hoist monorail
did
not have a rated load test conducted and maintained on file and no recorded
annual inspection had been conducted and maintained.
Among other methods, several feasible and acceptable abatement methods to
correct
these hazards are to:
1.Conduct a rated load test of the monorail and maintain the test reports
on
file in accordance with ANSI B30.11-1980, Section 11-2, Rated Load
test.2.Conduct frequent and periodic inspections of hoist monorail systems
and
maintain records in accordance with ANSI B30.11-1973 Monorail Systems
and Underhung Cranes, Section 11-2 Inspection, Testing, and Maintenance.
Abatement certification must be submitted for these items.
