a)On or about 9/17/09, Back Truck Shop:  A Rotary, four post surface
mounted lift was being operated with two of the locking mechanisms not
working, and with a hydraulic cylinder leak.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to:
Ensure the lift is operated in accordance with the Automotive Lift
Institute,
Inc. standard for automotive lifts, paragraph 4.4.2 which states; The
operator shall use all applicable safety features provided on the
automotive
lift, and operate the lift in accordance with the instructions furnished
by the
lift manufacturer.
Abatement certification must be submitted for this item.
