Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK-BY: a)Maintenance area, on or about 10/06/2009: An unknown, overhead hand operated chain hoist rated at 1/2 ton capacity was attached to a metal monorail attached to two ceiling beams and were not tested, rated, marked and no inspection, or maintenance records were maintained for the beam or the hoist. Among other methods, several feasible and acceptable abatement methods to correct this hazard are to: 1.Test, rate, and mark the monorail to ensure it will withstand the forces generated by the hoist and the rated load. ANSI B30.11.1-1980, Monorails and Underhung Cranes, Section 11.1 Construction and Installation. 2.Conduct frequent and periodic inspections of the monorail and maintain records in accordance with ANSI B30.11.2-1980 Monorail and Underhung Cranes, Section 11-2 Inspection, Testing, and Maintenance. 3.Conduct frequent and periodic inspections of the hoists and maintain records or use an external coded mark in accordance with ANSI B30.16-1993 Overhead Hoists, Section 16-2.1 Inspection. Abatement certification must be submitted for this item.
