Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK-BY: a)Paper storage area, on or about 10/22/2009: A freight elevator that was modified to an underhung hoist with a below-the-hook platform, 9-feet by 7 1/2-feet, attached to a 10 ton electric underhung hoist used to lift paper rolls ranging in weight from 500 lbs to 8000 lbs. was not tested, rated, inspected and no maintenance or modification records were maintained. b)Paper machine area, on or about 10/22/2009: A locally fabricated below-the- hook roll lifting spreader bar was being used to hoist, remove and install the bolt roll at the calendar roll system and the device was not tested, rated, marked or inspected with records maintained. Among other methods, several feasible and acceptable abatement methods to correct these hazards are to: 1.Test, Rate and mark the load capacity on the below-the-hook lifting device in accordance with ANSI B30.20-2006, Chapter 20-1, Below-The-Hook Lifting Devices. 2.Inspect and maintain records for all below-the-hook lifting devices in accordance with ANSI B30.20-2006, Chapter 20-1, Below-The-Hook Lifting Devices.. Abatement certification must be submitted for these items.
