Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck by the load should the overhead
hoist or supporting
structure fail:
a)Warehouse, Baler Room, on or about 3/18/10:  A one ton Coffing overhead
hoist had a missing safety latch, the load chain was binding when lifting
the
load block and hook and the hoist was mounted to an eye bolt through a hole
in a steel beam which had no rated load capacity markings on the structure
supporting the hoist.
Among other methods, feasible and acceptable abatement methods to correct
these
hazards are to:
1.Install a safety latch on the hook in accordance with ANSI B30.16-1993,
Section 16-1.2.9.
2.Ensure that the load chain runs freely and does not bind in accordance
with
ANSI B30.16-1993, Section 16-1.2.8.
3.Ensure that the supporting structure is designed to withstand the loads
and
forces imposed by the hoist for the rated load in accordance with ANSI
B30.16-1993, Section 16-1.3.2.
4.Inspect the hoist in accordance with ANSI B30.16-1993, Section 16-2.on
