29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  a) At the facility, 4529 Crown Road, Liverpool, NY., on or about 1/26/2023: Employees follow the respective energy control procedures while performing servicing and maintenance activities on equipment including, but not limited to, folding/gluing machine, die cutters, and printing press, having multiple energy sources (pneumatic and electric). The employer did not perform periodic inspections of the energy control procedures at least annually.

29 CFR  1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by in-going nip points, rotating parts, and/or flying chips and sparks.  a) At the facility, 4529 Crown Road, Liverpool, NY.; Die Prep Area. On or about 1/26/2023, employees use the Clarke Bandsaw to cut steel on a monthly basis. The bandsaw was not guarded to protect against the exposed saw blade to protect the employee's hand.
