29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter:  a)  At the establishment, on or about 1/5/16:  Annual training was not provided to employees on the use of fire extinguishers and the hazards involved with incipient stage fire fighting.  Training was last done May, 2014.  Abatement certification must be submitted for this item.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  a)  Mezzanine, on or about 1/5/16:  A strip outlet providing current to an EZ Drain unit for the Quincy QT30 air compressor was being used as permanent wiring.   b)  Mezzanine, on or about 1/5/16:  An extension cord providing current to the dryer for the air compressor was being used as permanent wiring.  c)  Printing Dept., on or about 1/5/16:  An extension cord providing current to a waste water pump was run along the floor under the Manroland printer and being used as permanent wiring.  Abatement certification must be submitted for these items.
