were causing or likely to cause death or serious physical harm to

were exposed to THE HAZARD OF BEING STRUCK-BY:  The load should the
automotive
lift fail.
a)At the facility, on or about 03/30/10:  A Forward, two post surface
mounted
lift was being operated without the rear arm locking mechanisms working.
Maintenance bay number four.
b)At the facility, on or about 03/30/10:  A record of all periodic
inspections of
automotive lifts was not prepared and/or maintained.
c)At the facility, on or about 03/30/10:  No documentation was provided
which
verified lift operators have been trained.
Among other methods, one feasible and acceptable abatement method to
correct this
hazards is to:
Ensure all automotive lifts are operated in accordance with the Automotive
Lift Institute, Inc. standard for automotive lifts, paragraph 4.4.2 which
states;  The operator shall use all applicable safety features provided on
the
automotive lift, and operate the lifts in accordance with manufacturers
instructions.  Additionally follow the guidance as specified in the
Automotive
Lift Institute, Inc. standard for automotive lifts, paragraph 5.4 which
states;
A record of each inspection shall be prepared and maintained noting the
observations and findings of all points of inspection recommended by the
manufacturer as well as subsequent repairs or replacements accomplished.
Also, ensure operator training is documented as specified in the Automotive
Lift Institute, Inc. standard for automotive lifts, paragraph 4.3 which
states;
The owner or employer shall appropriately document operator training by
completing the operator training log found in the appendix to this
standard.
Abatement certification must be submitted for this item concerning the
lack of
Qualified Inspection Documentation and the Operator Training
Documentation. .ocumentation.
