Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm:
Salt water disposal injection well operation.  On or about August 4, 2008,
employees operated
and maintained a high pressure salt water disposal injection well system.
Flexible hoses were
used to connect the pump and the well head to steel piping had no safety
restraints at connections
to prevent the hoses from whipping if a hose become disconnected under
pressure. This exposed
employees to the hazard of being struck by a whipping hose.  Among
feasible means one method
to correct the situation would be installation of steel cable restraints
manufactured specifically
for the situation.
