Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard(s) of electrocution while truck
drivers attempted to
drive an oversize load under energized overhead power lines.
At the site on 36th St. North, east of Havard Ave., Tulsa, OK: On or about
February 3,
2009, and at time prior thereto, David Stone Trucking Company allowed
truck drivers
transporting an oversize load  to approach within a safe distance of
overhead power lines.
As a result the load was caught on an overhead power line and other cable
and telephone
lines spread across the road.  The truck driver climbed up on top of the
load and used zipties
to tie the lines together trying to free the load from the lines.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
is to develop and implement a procedure for safely transporting oversize
loads under
energized overhead power lines and by contacting the utility company if
the height of the
load is 15'9" or more as required in the permit issued by the Oklahoma
Department of
Public Safety.

contacting energized
electrical lines.
At the site on 36th St. North, east of Havard Ave., Tulsa, OK and company
wide: On or
about February 3, 2009, and at time thereto, David Stone Trucking Company
allowed
employees to use lifting poles to lift energized overhead power lines
above and clear the
oversize load.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
is to develop and implement a procedure for safely transporting oversize
load under
energized overhead power lines.  The procedure should follow the
guidelines and instructions
of the Oklahoma Department of Public Safety and the Regulations of the
Oklahoma Office of
Secretary, Title 595, Chapter 30-3-17, Paragraph (e) or follow instruction
of other authority
having jurisdiction.

hazards associated with

Company allowed
truck drivers transporting an oversize load to approach within a safe
distance of energized
overhead power lines. The employer did not provide training to employees
related to the
procedures to be followed to minimize hazards associated with contacting
overhead power
lines.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
is to develop and train employees on the procedures to be followed when
approaching power
lines. The employer must maintain, review and upgrade these procedures as
needed.
