Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed the hazards of struck-by and crushed-by falling objects from
pull/drag loads,
falling objects from overhead installations and uncontrolled equipment
movement.
Demolition work at the powerhouse: On or about February 20, 2009, a Yale
forklift and a
pickup truck were chained together and used to horizontally pull/drag a
large boiler tank that
was chained to mast of the forklift. The boiler removal included all steam
stacks exposing
employees to unusual operating conditions such as overhead installations
and tipover.  The
employer failed to ensure employees were kept clear of loads, gave special
consideration to
overhead installations, used the forklift for vertical lifting only and
contacted the forklift
manufacturer for any conditions effecting the safe operation and capacity
of the equipment.
Among other methods, one feasible and acceptable method of abatement would
be to ensure
forklift operations are carried out in accordance with acceptable
practices set forth by the
equipment manufacturer and/or ANSI/ITSDF B56.1-2005.
