Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm:
Long #2 oil/gas well; on or about April 16, 2009, employees were engaged
in the operation of
a well service rig to replace the well down hole pump, they had pulled the
well sucker rod
storing it in the rig mast rod rack; the rig stabilizing guylines were not
attached to anchors leave
the rig stabilized only by rear carrier supports and guylines attached to
the carrier front.  This
exposed all the employees to the hazards of a rig over turning as being
struck by rig and well
components, and falling from mast elevations upwards of 80 feet.  The
American Petroleum
Institute (API) Recommended Practice (RP) 54, Recommended Practice for
Occupational Safety
for Oil and Gas Well Drilling and Servicing Operations 6.3.2 requires that
the rig be rigged up
in a safe manner before starting well operations with the rig, and 9.2.10
requires that the rig
guylines be properly tensioned prior to imposing a load on the derrick or
mast.  Among other
methods, one feasible and acceptable abatement method to correct this
hazard is to assure that
the rig is safely rigged up prior to the imposition of any operational
load on the rig mast and that
the guylines are properly tensioned to certified anchors.d

Long #2 well; on or about April 16, 2009, service rig employees were
engaged in the well work
on an oil/gas well to replace its down hole pump, this required that the
derrick hand be working
in the rig mast to manually place sucker rods into the mast rod rack; no
secondary means of
emergency egress was provided for the employee.  This prevent the derrick
from making timely
emergency egress from the mast during rig emergencies  with hazards as:
Fire, explosion,
flying objects and rig collapse.  The American Petroleum Institute (API)
Recommended Practice
for Occupational Safety for Oil and Gas Well Drilling Servicing
Operations, RP 54, 6.10.1
requires than an auxiliary means of escape be installed prior to the
derrick hand working on the
mast.  Among other methods, one feasible and acceptable abatement method
to correct this
hazard is to assure that the escape system is installed in accordance the
manufacturer's
instruction prior the placement of the derrick hand in the mast.
