Title 40.1-51.1.(A), Code of Virginia: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: (a) At this job site, approximately 100 metal form pins, being used in sidewalk concrete forming operations and protruding 4 to 15 inches above the forms, had not been guarded exposing employee's to the hazard of impalement. (b) At this job site, approximately 10 metal form pins, being used in sidewalk concrete forming operations, had mushroomed heads exposing employee's to struck-by hazards. "ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD, AMONG OTHERS, TO CORRECT THIS HAZARD IS TO INSTALL PLASTIC REBAR CAPS OR EQUIVALENT PROTECTION ON PROTRUDING FORM PINS AND EITHER GRIND OR CUT OFF THE MUSHROOMED HEADS".
