In that employees were using the same rubber hose, interchangeably between
the caustic soda
and sulfuric acid spigots, in order to transfer them to smaller
containers. These two chemicals
are incompatible and the resulting combination of the two, which is
feasible since some residue
from each chemical remained inside the rubber hose, can result in a
violent reaction that can
cause the chemicals to splatter and/or generate heat.
A feasible means of abatement would be to install different sized
connectors to each pipe spigot
which would ensure the hoses were not able to be switched and the
chemicals cross
contaminated.
