Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that an employee was exposed to a fall hazard while working from a ladder jack scaffold system without any means for fall protection. a. At the establishment located on 5170 S. Tamiami Trail, Sarasota, FL where an employee was standing on the platform of a ladder jack scaffold system 12 feet above the ground while performing routine maintenance work without fall protection, on or about 07/02/2008. Among other methods one feasible and acceptable method to correct this hazard is to provide employee with a personal fall arrest system that consist of an anchorage, connectors, a body belt or body harness and may include a lanyard, deceleration device, lifeline, or suitable combinations of these.ations
