Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer
did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of falling from a ladder:
(a) Location - Warehouse:
On or about 9/12/08, employees were exposed to fall hazards while using a
ladder
improperly.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard would be to: 1. Ensure that employees are trained in the safe use
of a ladder
consistent with manufacturers' recommedations and applicable version of
ANSI A14.5,
including but not limited to, not using fly section as a separate ladder.
2. Inspect ladders
prior to usage. 3. Do not over-reach while on a ladder.ach
