Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to the potential hazard of fire and explosion:
(a)West Side of Building (Outside) - The MAC dust collection system, model
number 144MCF255, used to collect class II wood dust from the planar, table
saws and sanders did not have a backflash damper, on or about June 17,
2009.
(b)West Side of the Building (Outside) - The exhaust air duct from the MAC
dust
collection system, model number 144MCF255, was recycled back into the
building during the winter months. The system was not equipped with an
isolation
device such as but not limited to a high-speed abort gate, on or about
June 17,
2009.
(c)North Side of the Building (Outside) - The dust collection
system(cyclone)
on top
of the storage bin did not have deflagration venting, on or about June 17,
2009.
ABATEMENT NOTE: Among other methods, one feasible and acceptable method to
correct the hazard is to comply with the National Fire Protection
Association (NFPA
664), Standard for the Prevention of Fires and Explosions in Wood
Processing and
Woodworking facilities"-2007 as it applies to your operations and
equipment including
but not limited to:
A.Equip the MAC dust collection system with a backflash damper, designed
and installed in accordance with National Fire Protection Agency(NFPA)
664.
B.Install deflagration vents on the cyclone in accordance with National
Fire
Protection Agency(NFPA) 664.
C.The exhaust air recycled back to the building shall be equipped with a
high-speed abort gate to intercept any burning material before entering the
plant as listed in Paragraph 8.2.2.6.2 of NFPA 664.
