Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to potential crushing injuries from a falling load while using an
inadequately engineered
lifting device.
(a)Injection Molding 1, Battery charging area - Employees used an
unlabeled lifting
device to lift fork lift batteries.  The employer did not insure that the
lifting
device was marked with its rated capacity, manufacturer information and
serial
number, on or about July 28, 2009.
(b)Industrial, Grid Casting - Employees used an unlabeled chain and bar
lifting
devices to lift various pigs.  The employer did not insure that the
lifting device
was marked with its rated capacity, manufacturer information and serial
number,
on or about August 2, 2009.
(c)Industrial, COS - An employee used an unlabeled lifting device to lift
various
pigs.  The employer did not insure that the lifting device was marked with
its
rated capacity, manufacturer information and serial number, on or about
September 15, 2009.
(d)Specialty Plant - An employee used an unlabeled lifting device to lift
various pigs.
The employer did not insure that the lifting device was marked with its
rated
capacity, manufacturer information and serial number, on or about October
21,
2009.
ABATEMENT NOTE:
Among other methods, one feasible means of abatement would be contained in
ASME
B 30.20-2003 Below the Hook lifting Device in that the company would have
an
engineer
determine the rated capacity and mark the information along with the
manufacturers
name and address, serial number, lifting device weigh if over 100 lbs on
the device.  The
device must also be inspected periodically and proof tested and certified
and insure that
the weight of the device is figured into the weight that can be lifted.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to the potential hazard of fire and explosion:
(a)Central Service - Air from the dust collector was recycled through the
duct work
back into the work areas with out the protection of a listed spark
detector system,
high speed abort gate, a reliable extinguishment system and/or a back flash
damper, on or about August 13, 2009.
(b)Central Service - Collection points for wood dust and other foreign
materials
including metal were not provided with magnetic separators, grate(s) or
other
types of screening to prevent foreign metal from entering the dust
collector, on
or about August 13, 2009.
ABATEMENT NOTE:
Among other methods, one feasible and acceptable abatement method to
correct these
hazards is contained in the National Fire Protection Association (NFPA)
664, "Standard
for the Prevention of Fires and Explosions in Wood Processing and
Woodworking
Facilities" (2007 edition)
* Section 8.2.2.6  Recycling Exhaust Air.  Air from air -material
separators or
dust collectors deemed to have either a fire or deflagration hazard shall
no be
recycled back into the building unless the provisions of 8.2.2.6.1,
8.2.2.6.2,
8.2.2.6.3, or 8.2.2.6.4 are met.
* Section 7.12.2 Foreign materials, such as tramp metal, capable of
igniting wood
waste and wood dust shall be prevented from entering the wood and dust
process
equipment.
* Section 11.1.5 Any metal collected through the cleanup process shall be
separated from wood debris or combustible waste to prevent entry into the
wood -
handling or processing equipment, the dust- collecting system, or the
scrap wood
hog.
