furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fire and explosion hazards as a result of the air material
separator (bag house)
not located outside, lack of isolation devices, mix area not segregated
from other areas, and lack
of controlling fugitive dust in mix area:
(a)Mixing Room - The mixing room, where employees mixed the facilitys
polyvinyl
chloride formulation (a powdered, combustible dust), was not segregated,
separated, or detached from the rest of the facility, on or about August
11, 2009.
(b)Mixing Room - The employer had the dust collector for the polyvinyl
chloride
formulation (a powdered, combustible dust) located inside the building, on
or
about August 11, 2009.
(c)Mixing Room - The employer had not installed isolation devices between
the dust
collector and the mixing equipment connected to the dust collector, on or
about
August 11, 2009.
(d)Mixing Room - Employees manually poured powdered components, including
calcium stearate and Rheolub, from bags into plastic garbage cans (used as
day
bins), hand scooped the powders from the garbage cans into plastic pails
for
weighing, and then hand dumped the powders into the mixer.  These hand
pouring, scooping, and dumping activities did not have ventilation to
minimize
the escape of combustible dusts, on or about August 11, 2009.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable abatement
method to correct this hazard includes following National Fire Protection
Association
NFPA 654, Standard for the Prevention of Fire and Dust Explosions from
Manufacturing, Processing, and Handling of Combustible Particulate Solids,
2006
Edition, including, but not limited to:
*Detach, segregate, or separate areas that produce, process, handle or
collect
combustible dusts from other occupancies to minimize damage from a fire or
explosion (NFPA 654, Chapter 6, Section 6.2).
*Locate dust collectors outside of buildings (NFPA 654, Chapter 7, Section
7.13.1.1.1), or protect dust collectors located inside of buildings using
oxidant
concentration reduction, deflagration venting to a safe location,
deflagration
pressure containment, deflagration suppression systems, dilution with a
noncombustible dust, or deflagration venting through a listed dust
retention and
flame-arresting device [NFPA 654, Chapter 7, Section 7.13.1.1.2(1) or (2)].
*Provide isolation devices, including, but not limited to, chokes, rotary
valves,
automatic fast-acting valve systems, flame front diverters, chemical
isolation
systems, etc., for dust collectors (NFPA 654, Chapter 7, Section
7.13.1.5).*Provide continuous suction to minimize escape of dust for
processes
that liberate
dust in normal operations (NFPA 654, Chapter 8, Section 8.1.1).
