(a) In the lathe department for the employee operating the Hwacheon HI-ECO
21HS
CNC lathe, serial #M058347C2KC, where the lathe was not equipped with an
operable
door interlock device exposing the employee to the potential hazard of
being struck by
parts being expelled from the turning lathe or being caught in the moving
machinery
inside the turning lathe by opening the lathe door during the machine
operating cycle.
(b) In the lathe department for the employee observed operating the
Hwacheon HI-ECO
21HS CNC lathe, serial #M058347B2KC, where the lathe was not equipped with
an
operable door interlock device exposing the employee to the potential
hazard of being
struck by parts being expelled from the turning lathe or being caught in
the moving
machinery inside the turning lathe by opening the lathe door during the
machine
operating cycle.
(c) In the lathe department for the employee observed operating the Index
G300 CNC
lathe, serial #500107, where the lathe was not equipped with an operable
door interlock
device exposing the employee to the potential hazard of being struck by
parts being
expelled from the turning lathe or being caught in the moving machinery
inside the
turning lathe by opening the lathe door during the machine operating
cycle.
(d) In the VMC Department for the employee observed operating the Monarch
CNC
mill, serial #91011B, where the mill was not equipped with an operable
door interlock
device exposing the employee to the potential hazard of being struck by
parts being
expelled from the mill or being caught in the moving machinery inside the
mill by
opening the mill door during the machine operating cycle.
(e) In the VMC department for the employee observed operating the Matsura
ES-550V
CNC mill, serial #031315562, where the mill was not equipped with an
operable door
interlock device exposing the employee to the potential hazard of being
struck by parts
being expelled from the mill or being caught in the moving machinery
inside the mill by
opening the mill door during the machine operating cycle.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to install a door interlock device on the above CNC lathes and mills as
required by ANSI B11.2-
2002 Safety Requirements for Turning Centers and Automatic, Numerically
Controlled Turning
Machines, paragraphs 6.21 Safeguarding and 6.21.1.2 Automatic Mode which
states:
1. Paragraph 6.21: Machines shall be safeguarded wherever practical by
fixed or
interlocked moveable guards.
2. Paragraph 6.21.1.2: Machine operation shall only be possible when the
guards are
closed and the safeguarding (protective) devices are active.  If it is
possible, in thismode, to open an interlocked moveable guard, the
hazardous movement shall cease and
be inhibited.
