Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards while being elevated on the forks of a Sky Trak Rough Terrain Forklift Truck. Date: On or about 11-06-2008 & approximately 10:00am. Location: On the North side of the block wall at the construction site located at 3123 Watson Blvd. Warner Robins, Georgia 31088. Condition: An employee was observed working from an elevated work platform approximately 10 - 12 feet above the ground level on a Sky Trak #3606 Rough Terrain Forklift Truck without a personal fall arrest system or guardrail system. The employee was using the Sky Trak rough terrain forklift to ride the forks and mask and a pallet to the top layer of the block wall. Among other means, one feasible and acceptable abatement method would be to provide a platform which complies with the design requirements listed in the American Society of Mechanical Engineers B56.6-1992 Section 5.15 Elevating Personnel. The employee was instructed to get down and not perform this unsafe operation again. Abatement was completed on the job site the day of the inspection 11-06-2008 No abatement certification or documentation is required for this item.
